Initial Comments:
Based on the findings of an off-site follow-up survey conducted on May 18, 2022, it was determined that Ark of Life Home Health, LLC., had corrected the following deficiency cited under Title 28 Pa. Code, Part IV, Health Facilities, Part IV, Chapter 611, Subpart H: Home Care Agencies and Home Care Registries. The deficiency was cited as a result of an on-site state re-licensure survey completed on March 21, 2022.
